Accessing Your ACA Form 1095 Under the Paperwork Reduction Act
In accordance with the Paperwork Reduction Act and IRS guidance, our employees may request a copy of their Form 1095-B (proof of minimum essential health coverage) directly from our plan administrator.
How to Request Your Form 1095-B
If you would like to receive a copy of your Form 1095-B, you may request it at any time by contacting:

Please include your full name, current mailing address, and the calendar year for which you need the form. Upon receiving your request, a copy of Form 1095-B will be provided to you within 30 days, as permitted under the Paperwork Reduction Act.
Electronic Availability
The IRS no longer requires us to automatically mail Form 1095-B unless requested. However, employees may access additional information about ACA reporting requirements directly from the IRS at IRS.gov.
